Tūranga mahi: Kaitūhono-a-Hapori

We are on the lookout for a new Kaitūhono ā-Hapori (Community Connector) to actively support individuals and whānau by connecting them with health services, social services, government, and non-government support services.

 The Kaitūhono ā-Hapori will provide direct support to whānau enabling their access to information and services in the community. You will work closely with our Iwi Work Brokers and community service providers to ensure that whānau do not miss out on entitled support and services that could adversely impact their health, wellbeing and future.

 This is a varied and rewarding position and can be located in Nelson or Tasman. For ore information, see our website.

 

While we are seeking full-time hours, we are willing to negotiate for the right person.  

You will be primarily responsible for:

• Determining the needs of whanau;

• Co-ordinating and facilitating access to services;

• Referring people to appropriate agencies and making appointments with service providers;

• Collaborating with local community groups, organisations and government agencies on initiatives and community events;

• Co-ordinating, facilitating or participating in agency meetings which promote government services;

• Providing support-related information and advisory services to people.

 

You will have demonstrated knowledge of community, government and local sector networks across the rohe, experience working with whānau in this kaupapa, have an excellent communication style (both written and oral), have an understanding of te Reo me ona tīkanga Māori, and the ability to build sound and trusted relationships.
